About the Medical Home Portal Archive
The Medical Home Portal website was initiated in 2001 to improve the care of children with chronic conditions and special health care needs. The extensive website grew tremendously, with over a million visitors in 2022, providing evidence-based clinical guidance for primary care of rare and common conditions, local and national resources for families and case managers to improve care, and support and information for families of children with complex care needs. The website partnered with several states over the years to tailor resources and information to local settings.
Unexpectedly, in 2024 the website was hacked and unable to be repaired without great expense. The decision was made to archive the content through the University of Utah’s Spencer S. Eccles Health Sciences Digital Library in collaboration with the University of Utah’s Department of Pediatrics. You can access this rich content by clicking on the links below to search the archive.
